Our contact form is provided by SimplePractice, a HIPAA-compliant practice management platform. When you submit the form, your name, email, phone number, and message are transmitted to SimplePractice and forwarded to our practice. SimplePractice's handling of this data is governed by their own privacy policy at simplepractice.com/privacy.
